Insurance:   Connie's services as a certified, licensed speech and language pathologist are often covered under voice evaluation, or speech therapy. Connie maintains her Certificate of Clinical Competence through the American Speech and Hearing Association and state licensure.  Itemized estimates or invoices will be provided upon request for insurance submission for reimbursement. Evaluation of voice is coded as 92524 and 92520. The evaluation is 2 hours and cost is $250.00. Voice therapy is coded as 92507 and is $125.00 per hour. 

 

    Estimates for precertification available upon request, but please direct your request in advance of the service date. We do not file or process insurance claims. Please check your individual insurance plan for any prior authorization or physician’s orders they may require.  You will be responsible for obtaining your prior authorization if needed. 

 

    Connie is not currently enrolled in Georgia Medicare and is unable to treat Medicare patients. Medicare does not allow an opt out to accept private pay for voice therapy.
